Braces: choosing between brackets, Invisalign, or lingual braces?

Three main options in modern orthodontics

Braces are an orthodontic procedure that repositions teeth, improves chewing, and creates an even, attractive smile. Today, patients have three main options: bracket braces (metal or ceramic), clear Invisalign aligners, and lingual braces. Each method has its own advantages and disadvantages, suited to different needs and circumstances.

Bracket braces

Bracket braces are the traditional and most common method, using brackets attached directly to the outer surface of each tooth, connected by an archwire. The brackets can be made of metal or ceramic.

  • Advantages: Highly effective for treating complex bite misalignments, cheaper than other methods (25-60 million VND), and good force control for precise tooth movement.
  • Disadvantages: Clearly visible when smiling, causes discomfort and mild soreness after each wire adjustment, harder to clean since the brackets and wires trap food and plaque, and requires avoiding certain hard or sticky foods.

Clear Invisalign aligners

Invisalign aligners are clear trays custom-made for each patient, fitting snugly over the outer surface of the teeth. Patients need to switch to a new set of aligners every 2-4 weeks.

  • Advantages: Nearly invisible when worn, easy to remove for eating and cleaning, making oral hygiene easier, more comfortable, less painful than brackets, and treatment effectiveness comparable to brackets.
  • Disadvantages: Much higher cost (45-120 million VND), may not be suitable for very severe misalignments, and requires the patient to be disciplined about wearing the aligners for enough hours each day (20-22 hours).

Lingual braces

Lingual braces are brackets attached to the inner (tongue-facing) surface of the teeth, making them completely invisible from the outside. This is a relatively new method in Vietnam.

  • Advantages: Completely invisible from the outside, highly effective for treating complex cases, and suitable for people who need to maintain a professional appearance.
  • Disadvantages: The highest cost of the three methods (usually from 60 million VND upward), harder to clean due to their position inside the mouth, may irritate the tongue and make speaking more difficult at first, and requires highly specialized skill from the orthodontist.

Detailed comparison

  • Aesthetics: Invisalign and lingual braces are both better than metal brackets. If aesthetics is the top priority, Invisalign or lingual braces are the best choices.
  • Comfort: Invisalign causes the least pain because the force applied is gentle. Brackets and lingual braces can irritate the soft tissue in the mouth.
  • Cost: Brackets are the cheapest, followed by Invisalign, then lingual braces.
  • Effectiveness: All three methods are similarly effective when performed by a qualified doctor. The main differences lie in treatment time, comfort, and aesthetics.

Choosing the right method

Choosing an orthodontic method depends on many factors: the severity of the misalignment, budget, aesthetic concerns, willingness to maintain care, and lifestyle. Patients should consult an orthodontist to get personalized recommendations based on their specific situation.

Treatment duration

Orthodontic treatment usually lasts from 18 to 36 months (1.5 to 3 years), depending on the severity of the misalignment and the method used. Invisalign may be slightly shorter for mild cases. Being transparent about the timeline from the start helps patients prepare mentally and financially.
